Fixed: PvP was still possible in PvP-deny regions in certain setups.

Fixed: Combat-log punishments could trigger death rewards, rankings and a duplicate death message on the synthetic death.

Fixed: An issue with the /dc menu.

Fixed: A misspelled or unknown material in a custom recipe could abort the whole plugin startup — it is now reported in the console and skipped.

Changed: Improved database schema migrations and index creation.
